Since 1988, the Houston Area Continuity of Care organization has united professionals in the senior care industry – to network, to learn and to reach out to Houston’s senior community. Whether it is members from elder law, in-home care, home health care, hospice, independent living/assisted living/memory care, financial planning, senior services, hospitals, clinics, or non-profit work focused on seniors, Houston Area Continuity of Care professionals are there – dedicated, educated, committed to offering only the best services available. Our seniors deserve nothing less.

CHARITABLE OUTREACH

The board of Houston Area Continuity of Care has established a discretionary fund that is targeted to help local not-for-profit organizations serving Houston’s seniors through a monetary gift. 


A call for nominations of eligible organizations is made to the membership each fall. A list of qualified nominations is compiled by the Member-At-Large, Discretionary Fund and presented to the membership for a vote. The winning organization, determined by a majority vote of the membership, will be notified. The presentation of the gift will be made by the Member-At-Large, Discretionary Fund at the January meeting.
