Houston Area Continuity of Care encourages information sharing among healthcare professionals dedicated to the continuity of care for senior adults. The organization provides member benefits through networking, education, and open communication -- allowing members to remain up to date on the evolving healthcare industry.
Since 1988, the Houston Area Continuity of Care organization has united professionals in the senior care industry – to network, to learn and to reach out to Houston’s senior community. Whether it is members from elder law, in-home care, home health care, hospice, independent living/assisted living/memory care, financial planning, senior services, hospitals, clinics, or non-profit work focused on seniors, Houston Area Continuity of Care professionals are there – dedicated, educated, committed to offering only the best services available. Our seniors deserve nothing less.
CHARITABLE OUTREACH
The board of Houston Area Continuity of Care has established a discretionary fund that is targeted to help local not-for-profit organizations serving Houston’s seniors through a monetary gift.
A call for nominations of eligible organizations is made to the membership each fall. A list of qualified nominations is compiled by the Member-At-Large, Discretionary Fund and presented to the membership for a vote. The winning organization, determined by a majority vote of the membership, will be notified. The presentation of the gift will be made by the Member-At-Large, Discretionary Fund at the January meeting.
